Teaching NeuroImage: Abducens Nerve Palsy With Ipsilateral Excessive Eye Tearing Attributed to an Internal Carotid Artery Sympathetic Plexus Schwannoma

نویسندگان

چکیده

A 65-year-old man developed subacute horizontal diplopia due to left abducens nerve (AN) palsy and excessive eye tearing. Brain MRI revealed a hyperintense T2 lesion with an elongated course within the carotid canal, presenting homogenous contrast enhancement (figure 1). The imaging findings were characteristic for internal artery sympathetic plexus (ICSP) schwannoma compressing AN. Subsequent irritation of deep petrosal originating directly from ICSP continuing as Vidian may have led lacrimal gland edema tearing 2). Thorough case presentation figure demonstrating relevant anatomy are available Dryad at [doi.org/10.5061/dryad.pzgmsbck6][1]. [1]: https://doi.org/10.5061/dryad.pzgmsbck6

Internal carotid artery dissection and pseudoaneurysm formation with resultant ipsilateral hypoglossal nerve palsy

Craniocervical artery dissection is a potentially disabling condition caused by an intimal tear allowing blood to enter and dissect the media in the cranial direction which can occur spontaneously or as a result of trauma. When the dissection extends toward the adventitia, it can form a protrusion from the weakened vessel wall called a pseudoaneurysm, which may become a nidus for distal thrombo...

Imaging of carotid canal sympathetic plexus schwannoma.

One case report of a schwannoma involving the sympathetic plexus in the carotid canal has been reported previously. This article presents 2 additional cases of this rare entity with associated clinical and unique radiographic findings. All patients presented with diplopia among other symptoms, and they had enhancing masses that smoothly expanded the carotid canal on cross-sectional imaging. Com...

Isolated hypoglossal nerve palsy due to internal carotid artery dissection.

A case of an isolated hypoglossal nerve palsy is reported. The differential diagnosis is discussed, in the context of the requirement for careful scrutiny of the entire course of the hypoglossal nerve on imaging, to detect underlying pathology remote from the tongue, and to avoid unnecessary invasive diagnostic procedures prompted by the appearance of a 'pseudomass' of the weak tongue both clin...

ANCA-Negative Granulomatosis with Polyangiitis Presenting with Hypertrophic Cranial Pachymeningitis, Abducens Nerve Palsy, and Stenosis of the Internal Carotid Artery

We report a rare case of granulomatosis with polyangiitis (GPA) presenting with hypertrophic cranial pachymeningitis (HCP), abducens nerve palsy, and stenosis of the internal carotid artery (ICA). A 59-year-old Japanese man presented with a year history of nasal obstruction and a 2-month history of slight headache. Histopathological examination of the granulomatous mucosa in the ethmoid sinuses...

Abducens nerve palsy as initial symptom of trigeminal schwannoma.

The insidious onset and slow progression of symptoms may cause delay in the recognition of trigeminal schwannomas. Two cases with abducens nerve palsy as an initial symptom are reported, in both of whom the tumour was growing mainly below and anterior to the intracavernous portion of the internal carotid artery. Early detection of clinical symptoms and thorough radiological investigations are i...
